Hazard/Safety Concern Notification
The Safety Office is charged with the responsibility of conducting periodic inspections of the BYU-Idaho campus and conveying safety information to respective faculty members or staff employees. This report is intended to help enhance safety awareness and is used at the discretion of the recipient.
Students and employees are encouraged to report hazard or safety concerns by submitting a Hazard/Safety Concern Notification. If you see or hear something that could harm someone, cause damage to property or may have an environmental impact, please complete and submit a notification.
Do not use this form to report an emergency or an immediate life threatening hazard - call 911.
Do not use this form to report a work-related injury or illness. Report employee injuries or illnesses to the Safety Office by submitting a Work-related Employee Injury report or by calling the Safety Office at (208) 496-5603.
